You should not fill form 15G for PF withdrawal, only fill part 1 of the form 15G. However when uploading form 15G on a UAN member portal (for form 19) upload both part 1 and part 2 pages of the it.

What is Actually Part 2 of Form 15G
Part 2 of form 15G contains the details of your regional PF office from whom you are getting your PF amount.

Why You Don’t Need to Fill Part 2 of Form 15G for PF Withdrawal
As an EPF member you are receiving the money when make a PF withdrawal request. For that only part 1 of form 15g should be filled, and the part is filled by regional office.
