PF name change form is also known as PF joint declaration form, you can use this form to correct your details in PF like your name, father / husband name, date of birth, date of joining, date of leaving etc.
If your Aadhar is already linked to your PF and if you want to correct your name then you have to submit a PF joint declaration form to your regional PF office.
How to Change/Update Name in EPF
There are Two ways you can correct your name in an EPF account. One is an online method and the other one is an offline method.
If your Aadhar is not yet linked to your PF account then you can use an online method, if your Aadhar is already linked to your PF account then you have to follow an offline method.
1. How to change name in EPF online
Step 1: Go to the UAN member portal and login with your UAN and password.
Step 2: Now go to the manage option in the menu bar and select the basic details option from the sub menu.
Step 3: Now you can find an option to correct your details.
Step 4: Enter your name, dob as per your Aadhar and also enter your Aadhar number.
Step 5: After entering all the required details click on submit.
Step 6: Within 20 days your PF details will be updated as per your Aadhar card, and that aadhar number will also be linked to your PF account automatically.
2. How to change name in EPF offline
It is useful to the EPF members who change their aadhar details, even if it was already linked to their PF account. In that case we can’t find the basic details correction option in the UAN member portal. And you will get the following message:
“ Aadhaar is already verified. Your details aren’t editable.”
In that case download the above PF name correction form and write your correct name, and wrong name as in your PF account, and attest this form with your employer and submit it to your regional PF office.
Along with this PF name change form submit your Aadhaar card copy and it will take a maximum of 20-30 days to correct your name in your PF account.
By following the same process and same form you can also correct yoru father’s name, or husband name, dob, DOJ, DOE etc..
